Annual Requirements for Student-Athlete Eligibility

Members of Susquehanna's cross country team run in a race.Each year, varsity student-athletes must complete NCAA compliance certification and institutional processes before participating in any Athletics activities, including strength and conditioning, practices and competitions.

CHECKLIST OF TASKS
Before diving into the tasks, review this checklist of annual required items. Task instructions and links are below.  

All Student-Athletes (first-year, transfer and returning)
Complete tasks in SportsWare account (NEW students must create an account; instructions below)
Complete the ImPACT Baseline Concussion Assessment
Complete NCAA and Susquehanna compliance tasks through FrontRush
Attend mandatory compliance session  

New Student-Athletes (first-year, transfer) 
Complete tasks for all student-athletes (above)
Complete the Student Health and Wellness Reporting Form (physical) and other requirements in the Admission River Hawk Portal

Who must complete: All varsity student-athletes (requirements vary for new and returning students)
Task opens: June 1
Due: July 5

Returning Student-Athletes

  • Update information if any changes from last year.
  • Complete and electronically sign the Annual Health Review.
  • Review and electronically sign the Medical Clearance Form (multiple signature lines).
    • Permission for Medical Release, Acknowledgment of Risk & Informed Consent, Student-Athlete Concussion Statement

NEW Student-Athletes (first-year, transfer)

STEP 1 — CREATE AN ACCOUNT
Request a new account. Use "Susquehanna" to create a new account, even if your previous institution used SportsWare (allow 1–2 days for approval). 

  • Complete following sections: General (use anticipated college graduation year – NOT high school), Address, Emergency, Insurance (upload front and back images of your insurance card).
  • Review and electronically sign Medical Clearance Form (multiple signature lines).
    • Permission for Medical Release, Acknowledgment of Risk & Informed Consent, Student-Athlete Concussion Statement

STEP 2 — PHYSICAL EXAM (RIVER HAWK PORTAL)
If you haven't already done so, log into your Admission River Hawk Portal. There are two online parts: 

  • Complete and submit the Student Health and Wellness Reporting Form physical evaluation – for NCAA participation, evaluation must be completed within 6 months prior to starting Athletics activities – and with your provider checking "CLEARED for full activity."
  • Complete the self-reporting electronic form, which includes Student Information, Personal Medical History, and Tuberculosis Risk Assessment sections.

STEP 3 — PHYSICAL EXAM (SPORTSWARE)
Because medical records are not shared from the Student Health Center, provide Athletics an additional copy: 

  • After you've uploaded your physical/health assessment form on the River Hawk Portal, upload the same PDF to SportsWare via the attachment section.

STEP 4 — SICKLE CELL DOCUMENTATION
Upload your Sickle Cell Trait documentation via the SportsWare attachment section. 

  •  If you don't already have it, use one of these ways to obtain documentation of your Sickle Cell Trait test results (associated costs are your responsibility):
    • Schedule a test through the NCAA's partner sicklecelltesting.pwnhealth.com;
    • Schedule a sickle cell trait test at a local diagnostics lab, which may require a physician's prescription; or
    • Request a copy of your newborn sickle cell trait status from your hospital of birth or pediatrician (allow six weeks).

ImPACT Baseline Concussion Test

Who must complete: All varsity student-athletes
Task opens: Now
Due: July 5 

Complete an ImPACT baseline assessment following the below instructions to ensure accuracy and validity. (under 45 minutes) 

Prepare testing conditions.
  • Be well-rested and not under the influence of alcohol or substances.
  • Find a quiet and distraction-free testing area.
  • Sit at a table or desk with a flat hard surface.
  • Use a computer or tablet (landscape mode) plugged into an electrical outlet.
  • Turn off pop-up blockers and close other programs and browser tabs.
  • Put phones, smart watches and other devices in airplane mode and move them away from you.
  • Use the same pointing device (e.g., external mouse, trackpad or game controller) throughout the test.
Login: Access the test here.
  • Enter customer code: 0454ad9018
  • Click "Validate" then "Launch Test" and follow instructions to complete the test.
  • DO NOT email or upload your Confirmation Certificate. After your test has been completed, the results will be sent to Sports Medicine staff for review.
  • If your score is deemed invalid, you must retest.

FrontRush Account Compliance Policies and Forms

Who must complete:
All varsity student-athletes
Task opens: Following on-campus meeting with compliance director
Due: Prior to first day of class

In addition to annual review of Susquehanna's Student Handbook (Code of Student Conduct), Course Catalog, and Residence Life and Community-Living Policies, student-athletes must review the following policies and complete the required NCAA and Susquehanna compliance forms online through FrontRush before signing each one.

Mandatory Compliance Meeting

Who must attend: All NCAA student-athletes
Due: TBD – Your coach will provide the date, time and location. 


NCAA Amateurism Certification for New International Student-Athletes

Who must complete:
All international college-bound student-athletes (first-year and transfers) who: 

  • Have a permanent residence outside the U.S.
  • Attended secondary (high school) or postsecondary school outside the U.S. (excluding U.S.-based students studying abroad).
  • Participated on a sports team or competed as an individual athlete based outside the U.S. 

Task opens: April 1
Due: Prior to first day of class

Since final confirmation can take several weeks, request your amateurism certification early through the NCAA Eligibility Center for each Division III sport you plan to participate in. Amateur certification ensures you have not received payment for athletic participation other than approved Name, Image and Likeness (NIL) deals. Here is a registration checklist: 

Login: Register or access your account here and choose Amateurism-Only Certification.

  • You can use an existing Academic and Amateurism Certification account.
    • A fee is required, unless approved for a fee waiver.
  • Provide information about yourself, your education history and your sports participation history. Submit supporting documentation as requested.
  • Request your final amateurism certification for each sport to the Eligibility Center.
    • Provide Susquehanna with your NCAA ID. The Eligibility Center may hold your certification until after you are added to Susquehanna's request list.
